Will microneedling actually fix my acne scars?

Microneedling works well on rolling and boxcar acne scars, which respond to collagen remodeling. Deep ice-pick scars improve partially but rarely resolve fully with microneedling alone, and may need a combined plan with TCA CROSS or subcision. We assess scar type during your first visit and tell you honestly which scars will respond and which probably won't. Plan on a series of 4 to 6 sessions spaced 4 to 6 weeks apart for acne scar revision.

How soon can I surf or swim after microneedling?

Wait 5 to 7 days before ocean or pool swimming. Saltwater, chlorine, and unfiltered sun exposure on micro-channeled skin can cause irritation, pigment changes, and infection risk. Most San Diego patients book their session on a weekday and resume surf or beach time the following weekend with SPF 50 mineral sunscreen and a hat.

Is microneedling safe for melasma given how much sun exposure San Diego has?

Exosomes are lab-purified messenger vesicles (we use Benev) that signal aggressive regeneration and reduce downtime by roughly half. PRP is the more proven option with decades of clinical use. Exosomes are newer, more potent for downtime reduction, and the better pick if you are post-laser or have inflammation-driven concerns. How long do microneedling results last?

A single microneedling session produces a glow that peaks at 4 to 6 weeks and gradually diminishes over 3 to 4 months. The collagen remodeling from a full series (3 to 6 sessions) lasts much longer: most patients hold their improvement for 12 to 18 months before booking maintenance. Maintenance is typically one session every 4 to 6 months.

When should I not get microneedling?

Skip microneedling if you have an active acne breakout in the treatment area, active cold sores or other skin infections, recent isotretinoin (Accutane) use in the last 6 months, a history of keloid scarring, are pregnant or breastfeeding, or have an immunocompromising condition. We screen for all of these at the first visit. We also do not treat over Botox or filler that was placed less than 2 weeks prior.

The right starting point for general texture, pore refinement, prejuvenation in your 20s, and maintenance after a completed PRP or Exosome series. Topical hyaluronic acid and vitamin C are pushed into the micro-channels during the pass. Lowest downtime of the three, typical glow at 7 to 10 days.

PRP Microneedling

The right choice for acne scar revision, deeper anti-aging, and patients who want the most clinically proven boost. We draw blood in-clinic, centrifuge it in front of you, and apply the platelet-rich plasma through the channels. Fully autologous, zero allergy risk. Arrival and intake (10 minutes)

Check in, review skincare and medications you took in the last week, confirm any sun exposure, and take baseline progress photos in consistent lighting. If this is the PRP tier, your blood draw happens here so the centrifuge can run during numbing.

Topical numbing (20 to 30 minutes)

Prescription-strength lidocaine is applied across the treatment area and covered with occlusive wrap. You sit in a quiet room with water. The numbing is deeper than over-the-counter creams and reaches the dermal layer the needles will touch. You feel pressure and vibration during the pass, not sharpness.

The microneedling pass (45 to 60 minutes)

Numbing is wiped off. The DP4 glides across the face in overlapping passes at the depth chosen for your goal: 0.5 to 1.0 mm for pore and texture work, 1.0 to 1.5 mm for fine lines, 1.5 to 2.5 mm for acne scar revision. Serum (Signature), PRP, or exosomes are applied between passes so they absorb through the channels. Neck or décolletage add-on adds 10 to 15 minutes.

Cool-down and post-care (10 minutes)

A hydrating mask cools the skin while we walk you through aftercare. You leave with a post-care kit (gentle cleanser, mineral SPF 50, recovery serum) and your 2-week follow-up scheduled. The face feels like a moderate sunburn at this point, redness is at its most intense, and you will want to drive home wearing your sunglasses.

Recovery Day by Day

Recovery from microneedling is more predictable than most patients expect, but the rules are specific. Skipping them is what causes the bad outcomes you read about. The timeline below is for a standard face treatment. Exosome sessions cut redness roughly in half. Acne scar work at deeper settings adds 1 to 2 days.

Day 0 (treatment day)

Face is red and warm like a moderate sunburn. Mild swelling possible in the cheeks. No active skincare, no makeup, no exercise that flushes the face, no alcohol. Sleep slightly elevated on a fresh pillowcase.

Day 1

Redness softens to pink. Skin feels tight, possibly slightly itchy as it heals. Mineral SPF 50 minimum any time you leave the house. Gentle cleanser only. Hyaluronic acid serum is allowed. No retinol, no AHA, no BHA, no vitamin C yet.

Days 2 to 3

Mild flaking or peeling may begin, especially around the mouth and forehead. Do not pick. This is old surface skin shedding and is normal. Makeup is safe at this point if you want it. Workouts that don't heavily flush the face are okay.

Day 5 to 7

Skin looks calm and bright. You can resume retinol, vitamin C, and gentle acids. Ocean and pool swimming are safe at the 5 to 7 day mark with mineral SPF. Hot tubs and saunas are also okay now. The early glow starts to show.

Weeks 2 to 4

Glow peaks. Pores look smaller, makeup sits better, texture feels smoother. This is when collagen remodeling is most active under the surface even though the visible improvement plateaus.

Months 2 to 3

Collagen continues to build. Acne scars soften, fine lines look less etched. If you booked a series, your second and third sessions land in this window (typically 4 to 6 weeks apart).

Month 6

Peak collagen response from a single session. If you completed a full series, this is when the final 'this is what I paid for' result is visible. Maintenance is one session every 4 to 6 months from here.

How to Vet Any Microneedling Provider in San Diego

Microneedling is a medical procedure with real risk if it's done wrong. Pigment changes, infection, and uneven scarring from poor technique are all genuine outcomes you can find documented in dermatology literature. Whether you book with us or anywhere else, the questions below are worth asking before any San Diego clinic puts a device on your face.

Who is performing the treatment?

Ask whether your provider is an RN, NP, PA, or aesthetician, and what their scope of practice is in California for this device. Depth control on facial skin is a clinical decision. We perform microneedling with RNs only.

What device is being used?

Ask for the device by name and confirm it is FDA-cleared. Generic 'derma pens' are a red flag. We use the DP4 Dermapen, the only fully digital microneedling pen on the market. Digital control prevents the dragging and uneven puncture depth that analog pens cause.

Are the needle cartridges single-use and sterile?

There is one acceptable answer: yes, single-use, sealed, opened in front of you. Reused cartridges are a documented cross-contamination risk. If a clinic cannot answer this clearly, do not book.

Is PRP drawn and processed on-site?

Real PRP requires your blood, drawn that day, spun in a centrifuge that day, and applied immediately. If a clinic offers 'PRP' without an on-site draw, you are getting a growth factor serum, not PRP. Both can be reasonable, but they are not the same product.

What exosome product are they using?

Real exosome products have a manufacturer name and a published source. We use Benev. Other clinically credible names you may see are ExoCel and AnteAGE. Unlabeled vials, 'proprietary' exosomes with no source disclosed, or pricing far below market are reasons to ask harder questions.
